Maryland has joined with five other states in participating in the AHEAD Model which will promote health care through greater care coordination and focus on health equity and social needs for underserved patients. The Centers for Medicare and Medicaid Services Administrator Chiquita Brooks-LaSure said Maryland has already been driving innovation through its Total Cost of Care Model and all-payer hospital payment system…
“For millions of Marylanders, the AHEAD model will mean stronger primary care, stronger connections to community resources, and more coordinated care that supports all of their healthcare needs. It will mean not getting shuffled around and treated as a diagnosis, but instead be supported and cared for as a person.”
