Health Dept, UPMC Roll Out Vaccine Interest Form

The Allegany County Health Department and UPMC Western Maryland have announced the launch of an online COVID-19 Vaccine Interest Form, available to all Allegany County residents.  The purpose of the form is to allow residents to submit their name, age, email address and phone number so that – when Allegany County reaches their priority group and vaccine is available – either the hospital or the health department can contact them to schedule a vaccination appointment. 

“We are excited to be able to streamline the COVID-19 vaccination appointment process in Allegany County.  The new online COVID-19 Vaccine Interest Form will allow community members to submit their information so they can easily be contacted by the Allegany County Health Department or UPMC Western Maryland when vaccination appointments are available for their priority group," said Jenelle Mayer, Health Officer for Allegany County.

A link to the interest form can be found at https://health.maryland.gov/Allegany.  It is important to note that the interest form is NOT an appointment scheduler, but rather a contact database which will facilitate COVID vaccine appointment scheduling in Allegany County.

On Wednesday, the Allegany County Health Department reported 13 new COVID cases in Allegany County and five additional deaths, bringing the county’s cumulative case count to 6,218 with 189 fatalities.  The current case rate in Allegany County is 20.08 cases per 100,000 population, while Maryland’s state average is currently 25.81 cases per 100,000 population.  Allegany’s positivity rate is at 4.82%, while the statewide average is 5.96%.
